This survey report is based on our analysis of 537 surveys received from US refractive and cataract surgeons.

Weak economic conditions and weak consumer confidence resulting from high gas prices, rising food prices and concerns over high levels of unemployment continued to reduce demand for US refractive surgeries during the second quarter of 2008. Economists forecast continued economic weakness for the balance of 2008 and into 2009, and this troubled outlook is expected to continue limiting demand for refractive procedures.

IOL procedure volume grew 3.2 percent in Q2-2008 when compared with the same quarter last year. Refractive lens exchange procedures grew at a much more robust rate, buoyed by the increasing popularity of presbyopia-correcting IOLs, and phakic IOL procedures also grew briskly.

Collecting customer feedback and taking action to make improvements is a well recognized method for boosting performance. Customer feedback programs have become one of the most important tools for improving performance, and many of the most successful companies, including Apple, Amazon, Cost-Co, Lowes, and Harley Davidson, regularly measure their scores. Results from these surveys are analyzed in detail and these companies all have programs targeted to make continuous improvements.

In 2007’s fourth quarter, US intraocular lens surgeries (cataract, phakic, and refractive lens exchange) grew 3.2 percent over totals in the fourth quarter of 2006 and 1.3 percent over 2007’s third quarter tally.

Market Scope Gathers Guidance from Consultants, Surgeons and Practice Managers
In this issue of Market Scope’s Surgeons’ Quarterly Survey Report, we are taking a look at how refractive surgery practices are faring in the current period of economic uncertainty, as well as what strategies might help practices successfully weather tough economic times.
We asked Marguerite McDonald, MD, a pioneering specialist in corneal disorders and cataract and refractive surgery with Ophthalmic Consultants of Long Island, NY, and Robert Maloney, MD, also a pioneering refractive surgeon and founder of the Maloney Vision Institute in Los Angeles, CA, about their expectations, observations and plans for their own practices. Dan Wilson from the ClearSight Center in Oklahoma City, OK, also participated and brought a practice manager’s perspective to our questions.

Q1-2007 IOL procedure volume climbed 2.8 percent higher than in Q1-2006, as new IOL technologies took the lead in growth. Although short of early expectations, PC-IOL surgeries posted gains of more than 21 percent, while together RLE and phakic procedures were up more than 11 percent.

IOL procedure volume grew at a quicker pace in Q4-2006 than in the previous quarter, while PC-IOL volume again fell short of expectations. However, the new technologies boosted refractive lens exchange and phakic IOL implantations to brisk growth, although these procedures still represent a very small portion of total IOL procedure volume.

Whether it was the time of year or the tropical location, the Royal Hawaiian Eye Meeting (RHEM) was a hotbed of activity this year. Although many have been attracted to past RHEM meetings due to the location and relaxed pace, the meeting has expanded in recent years to include a more rigorous schedule and has joined the ranks of other important meetings. More than 1,000 doctors attended this year’s meeting, with approximately 800 registered for the general sessions and another 200 for the retinal program. Approximately 100 exhibitors showed their wares in the exhibit hall.

A January 22nd ruling by the Centers for Medicare and Medicaid Services (CMS) is expected to increase the use of toric IOLs in 2007. The CMS ruled that Medicare beneficiaries who undergo cataract surgery and choose a toric (astigmatism-correcting) IOL may be charged a premium above Medicare’s typical cataract reimbursement for the additional cost of the lens and the surgeon’s time. The ruling is similar to the 2005 ruling allowing a premium charge for presbyopia-correcting IOLs.
